Start with one inquiry you can follow
A form confirmation tells you that something happened on the website. It does not tell you that the right person received the inquiry, that the source was retained, or that the inquiry was suitable for your business. A useful tracking audit follows those steps separately.
Start with a single, agreed test. Give it an unmistakable test identifier and tell the receiving team what to expect. Use your own test contact details, keep it out of sales totals, and avoid sending sensitive information. Repeat the process for each materially different route: a main contact form, a campaign landing page, a phone number or a chat handoff.
Your first-pass check
Tick only what you have verified. This checklist stays in this page and does not send a test inquiry.
0 of 8 checks recorded. Unchecked items remain unverified.
Check five separate outcomes
- Action: did the visitor complete the intended step, and what appeared on screen?
- Receipt: is the inquiry present in the actual inbox, call log or CRM destination?
- Measurement: did the expected event appear once, with the correct definition?
- Routing: is a named person or team responsible for responding?
- Qualification: does the inquiry meet a documented business definition, and was that outcome recorded?
Record a timestamp, timezone, evidence reference and result for each step. Use “not tested” when you have no observation. If a step does not apply, explain why. A spreadsheet filled with ticks is less useful than a short record that shows where the chain breaks.
A completed example
Fictional example: test T-001 submits successfully and creates one analytics event. The receiving team cannot find it in the CRM. Mark the website action and event as observed, CRM receipt as unverified, and qualification as not assessed. The next task is to inspect delivery and routing—not to count T-001 as a qualified lead.
After the delivery problem is resolved, make a new dated test record. Keep the first result so you can explain what changed. Check that retries did not create multiple inquiries or multiple primary conversion records.
Separate source capture from a sales claim
Record the source information that actually arrived. An attributed channel can depend on consent, redirects, tracking setup and reporting rules. If the source is missing, keep it unknown until you can resolve it. Do not fill the gap with the channel you expected.
Google Analytics events describe interactions. Google Ads offline-conversion guidance explains measuring later business outcomes. Use the current account instructions for implementation; this worksheet helps verify your own journey and does not upload conversions.
Decide what to fix first
Put a lost inquiry ahead of a cosmetic reporting issue. Assign each unresolved item to a person who can inspect the relevant system, set a retest date, and retain the evidence of the new result. Check mobile and desktop journeys when their forms or call behavior differ.
Use the measurement framework to define your funnel, then the lead economics calculator once the counts and outcomes are trustworthy.
Take the worksheet with you
Use the blank worksheet for your own review. Any completed examples are clearly labeled as fictional.
